What Documents Are Required for the O-3 Visa Dependents?
The following documents are required for the O-3 Visa:
your valid passport;
one photograph that meets the US Visa Photo Requirements;
Your confirmation page for Form DS-160;
Receipts indicating that you have paid your fees;
The letter of confirmation for the interview;
Form I-797 of either the O-1 or O-2 visa holder;
The copy of the passport of the O-1 or O-2 visa holder,
Evidence of your relationship to the O-1 or O-2 visa holder;
